Traditionally, the medical staff operates apart from the hospital in such a way that the physician community views the hospital and staff as resources for services not available at their office; and the hospital sees the doctors as their sole source of revenue because no service can be delivered in a hospital without a physician order.
It Begins with Trust
Trust is an easy word to toss around, and most people can offer up a definition that makes sense for everyone in the room, but for business leadership purposes the definition requires some discussion.
We will define trust as a combination of courage, vulnerability, and transparency that causes leaders to take personal risk by stepping up to guide their team as needed. This kind of trust also fosters an environment where the team becomes self-guided and empowered, practicing at the top of their license in the best interest of optimal patient care.
Doctors must now become leaders
Although these lines have been blurred recently as more and more doctors have moved out of their independent practice and become hospital employees, the real change is still to come when the Fee for Service (FFS) reimbursement model is replaced by Value Based Reimbursement (VBR).
It will be important that physicians step into leadership roles and work alongside the other stakeholders (including hospitals, ancillary providers, employers, patients, healthcare payers, and the community) as the VBR model is defined and implemented, and clinical integration strategies are developed in local communities across the country. This Collaboration with their traditional “Supply Chain” is a new concept all stakeholders must embrace. This is critical from a patient safety and quality standpoint as well as the financial perspective.
IT REQUIRES COURAGE
Courageous leaders are needed in every area. With the imminent transition to value based reimbursement for healthcare services, leadership in the healthcare delivery space will require more courage than ever. This paradigm shift will require players who have been semi-autonomous historically to come to the table and collaborate like never before. This will require hardnosed dialog and unwavering commitment to building and executing in new delivery systems where all stakeholders are bought-in and will thrive in the overall success of the system. The winners in this evolution will be those who are prepared to roll up their sleeves, and let their guard down in the interest of finding solutions for fundamentally changing the way healthcare is delivered and compensated in the United States. Courageous leaders are needed in every area.
